1.______I agree to take care of this secondhand angel (dog) to the best of my ability.  I will provide fresh water, food, shelter, a healthy living environment, and love daily.

2.______I agree to get the dog to the veterinarian for shots, any sickness, etc. as needed.  I will spay/neuter my dog if at all possible. (If we control animal population, we would not have as many unwanted animals without homes.)

3.______I agree to bring the dog back to Secondhand Angels if the dog does not work out for whatever reason this may be.  I may even exchange the dog if possible.  I will not give the dog to anyone else without Secondhand Angels approval.

4.______I will notify Secondhand Angels if my dog becomes lost.

5.______I will treat my new angel as a member of the family.  Dogs are very sensitive and need attention at all times. 

6.______I will accept all responsibility for my dog and I release Mrs. Birdie Conant (Secondhand Angels Individual Rescuer) of any and all responsibility where this dog is concern once I sign this paper and take the dog from the premises.  I understand that these dogs where all rescued animals and that little may be known about this dog that I am helping and giving a “forever home”.   I understand that this dog has been abused, abandoned, or surrendered.

I fully and completely understand and agree to this agreement.  I have read and I understand everything that has been stated in this agreement and have asked questions when I needed anything clarified.

Please understand that these questions are asked just so I can better place my angels in to the perfect “forever home”.  I care very deeply for these dogs and they have put their trust in me to care for them the best that I can.  Please note that I will be checking with landlords to confirm that pets are allowed.  If your application is approved, and you take an angel home, and find that it is not a good match or that the dog is not what you were looking, for you must bring the dog back to me. DO NOT GIVE THE DOG TO SOMEONE ELSE.  I WILL TAKE HE OR SHE BACK.  This way I can try to find it a forever home somewhere else.

 

I AM AN INDIVIDUAL RESCUE PERSON!  THERE IS NO GROUP OR ORGANIZATION INVOLVED.  I DEPEND SOLELY ON DONATIONS TO SUPPORT MY SECONDHAND ANGELS AND TO PAY THE WORKERS WHO ARE VERY DEVOTED TO THEM.  DONATIONS ARE GREATLY APPRECIATED.
